New Patient Registration

docandcomputerThe Practice accepts patients living within the Practice Area (see below).  To register, please fill out a GPR new registration form (available at reception) for each member of the family wishing to register. Patients can also initially register online on the Practice website - www.dounehealthcentre.co.uk.  You will also be given a New Patient Information form for each member of the family and offered a New Patient Medical appointment for every member over five years of age.  If you are on repeat medication, then an appointment must be made with a GP when registering.

It is important to note that, unless you hand the signed forms back to reception, then you will not be properly registered and we will be unable to request your medical records from your previous GP.

All patients will receive care without discrimination, be given a full explanation where there is any undue delay in their care, receive a second opinion if requested and deemed reasonable, and be allowed to choose whether or not to participate in clinical trials and student training.  Patients have the right to express a preference for a particular practitioner by asking the Receptionist.

* Please note that where a patient is not registered with a GP practice for any reason, the period of non-registration may invalidate specific policies, e.g. a travel insurance policy.  Please check that you and anyone to be covered by the policy are:

  • A permanent resident of the United Kingdom
  • Registered with a Medical Practitioner in the United Kingdom
  • Travelling from and returning to the United Kingdom

You may print off a registration form & medical questionnaire, fill them out and bring them in with you on your first visit to the practice, otherwise you could email them to us at fv.gp25224douneadm@nhs.scot .
